What Are The Types Of Breast Augmentation ?

1. Breast Augmentation with Breast Implants

The typical method of breast augmentation involves inserting breast implants, either saline or silicone, into a prepared pocket in the breasts. Most often, the pocket is prepared underneath the pectoral muscle for added concealment and a more natural look.

Saline breast implants consist of a silicone shell that is filled with sterile saline solution (saltwater). They are inserted into the breast pocket while empty and then filled with the saline solution to the desired size. This means that they have slightly smaller incisions, as they are filled after they are implanted into the breasts. In the event of a rupture, the saline solution is harmlessly absorbed by the body. This makes ruptures easier to detect as the implant deflates, resulting in noticeable changes in breast size and shape.

Silicone breast implants also have a silicone outer shell but are filled with a cohesive silicone gel. The gel closely mimics the feel and consistency of natural breast tissue, which makes them preferred by patients who want a natural look and feel. However, silicone implants are pre-filled by the manufacturer, requiring a slightly larger incision for placement. In the event of a rupture, the gel tends to stay within the pocket made for the implant, making ruptures less noticeable. Therefore, regular monitoring, such as periodic MRI scans, is recommended to patients with silicone implants.

2. Breast Augmentation via Fat Transfer

Autologous fat transfer, which simply means “the patient’s own fat,” is another method of breast enhancement. Unwanted fat is removed through liposuction from other areas of the body such as the abdomen, thighs, back or buttocks. That harvested fat is then filtered and injected into the breasts to make them fuller. This creates a truly natural look and feel to the breasts, since the added volume is natural fat from the patient’s own body. Since the fat is a living substance, it has to establish a blood supply in its new location. Some of the fat will not survive and is absorbed by the body. Whatever you have in 3 months, you will keep.

How Can I Prepare For Breast Augmentation?

After completing your breast augmentation consultation, it is most productive to prepare for breast augmentation by planning for your recovery process. It's essential to arrange for someone to drive you to and from the surgery center on the day of the procedure and to have someone available to assist you at home during the initial recovery period. Create a comfortable recovery space in your home and stock up on necessary supplies such as loose-fitting clothing, button-up tops, ice packs, Gatorade, protein drinks and any other desired post-operative care items. You will be asked to quit smoking three weeks before surgery, as it can impede blood circulation and the healing process. If you have multiple medical conditions, Dr Dunkley may need an EKG and a note from your regular doctor stating you are healthy enough for surgery.

What Can I Expect On The Day Of My Breast Augmentation?

On the day of your breast augmentation surgery, your appointment will start with meeting the surgical team. Dr. Dunkley will speak to you and mark the breasts to guide surgery. Once in the operating suite, you are given anesthesia and will go to sleep. This ensures you will be safe and comfortable for the duration of the surgery, with no pain or memory of the surgical process. You will awake in the recovery room and stay until the anesthesia has worn off.

What Happens During Breast Augmentation Surgery?

Breast augmentation surgery will vary depending on whether you’re receiving breast implants or a fat transfer to the breasts, and may have additional variations depending on your personal treatment goals.

Breast Augmentation with Breast Implants

During breast augmentation with breast implants, Dr. Dunkley will use one of two incisions. The placement of the incision will be agreed upon by the two of you prior to surgery. The size of the incision varies slightly depending on the type of implants used, saline or silicone. If a lift is needed, the incisions will be different. For standard breast augmentation, your options include the following:

  • Inframammary incision: This incision is typically made horizontally along the crease underneath the breast. It allows for direct access to the breast pocket. It is commonly used for both saline and silicone implants.
  • Periareolar incision: This incision is made along the lower border of the areola, where the darker pigmented skin meets the lighter skin of the breast. This incision is well-concealed within the natural color transition of the areola. It allows for less visible scarring, but may not be suited for larger implants, particularly larger silicone implants.

Once incisions are made, the desired breast implants will be inserted into a pocket made under the breast tissue and the incisions will be closed.

Breast Augmentation via Fat Transfer

During a breast augmentation via fat transfer, treatment begins with liposuction to harvest unwanted fat from other areas of the body. This will require a small incision through which the fat is collected. Common areas to get fat from include the stomach, legs or back. After the fat is collected, it will be processed and prepared for injection. Finally, the fat is injected directly into the breasts to produce the desired results.

Can I Go Home After Breast Augmentation Surgery?

Yes, breast augmentation surgery is an outpatient procedure, so most patients are able to return home within a few hours of their surgery. However, you should arrange for someone to drive you home and stay with you during the first few days of your recovery. This ensures that you have assistance while the anesthesia effects fade and you begin your healing process.

How Long is Breast Augmentation Recovery?

From the day of your surgery until the day you are fully recovered is typically 1-2 months.. However, it’s important to note that the recovery process can vary from patient to patient depending on their health, adherence to post-operative guidelines and the extent of surgery performed. Furthermore, most people return to their normal routines well before they make a full recovery.

What is the Breast Augmentation Recovery Timeline?

Breast augmentation recovery will vary by patient, but the week by week recovery process will generally resemble the following timeline:

Week 1

Immediately after your breast augmentation surgery, you can expect some pain, swelling, discomfort and pressure in the chest area. Your breasts will be covered by a surgical bra or compression garment to provide support and reduce swelling. It's important to avoid strenuous activities during this time, however, you should be up walking during the day, not just laying in bed. Plan on taking time off of work and exercise.

Week 2-3

Swelling and bruising subside significantly. After one to two weeks, most patients return to work and light activities. Strenuous activities, including heavy lifting and exercise, will still need to be avoided.

Weeks 4-6

During this time, patients are able to resume most if not all of their normal activities, including returning to exercise, as they approach a full recovery. Most side effects should resolve by this time, although changes in sensation in the breasts and nipples may persist for two to four months. Results should become more apparent as the implants or injected fat settle into their final position.

Post Surgery Frequently Asked Questions

How Should I Sleep After Breast Augmentation?

After undergoing a breast augmentation surgery, you will need to sleep in certain positions to protect the incision areas and facilitate healing. Typically, we recommend sleeping with your head elevated to reduce swelling and enhance blood circulation. This can be achieved by propping yourself up using pillows or opting for a reclining chair, allowing you to sleep in a semi-sitting position for the first few days. It is important to avoid sleeping on your stomach during the initial stages of recovery to prevent strain on the chest tissue and incisions. Side sleeping if fine after 3-5 days, though alternating sides is recommended.

Can I Still Breastfeed After Breast Augmentation?

The majority of women can breast feed after having implants. The ability to breast feed can be affected by the choice of incision site. An incision in the crease under the breasts doesn’t go through any breast tissue, and is more likely to preserve your ability to breastfeed. A periareolar incision goes through your breast tissue and is more likely to affect your ability to breast feed. However, if breastfeeding is a priority of yours, it is important to discuss it with Dr. Dunkley during your consultation. In some cases, patients may benefit from prolonging their breast augmentation surgery until after they are no longer breastfeeding.

How Long Do Breast Implants Last?

Breast implants are designed to be long-lasting and carry a lifetime warranty. However, patients should expect to change their implants at some point during their life. Usually patients elect to change their implants because they become unhappy with the shape. As a breast ages it typically loses natural tissue and elasticity, meaning they start to droop. This process can be accelerated slightly with breast implants due to the extra weight they add to the breasts. As a result, many patients choose to have a breast implant revision or a breast lift later in their life. This timeline will be different for every patient It is normal to keep your breast implants until you are no longer happy with the shape of the breasts.
